Manual Submission

  1. Select the appropriate work order form: For Silicone Sockets and Liners use the ‘Si’ work order, For Carbon Frames use the ‘C’ work order.
  2. Download the 14ELMT Work Order form (PDF file) to your desktop.
  3. Open file in a PDF reader, like Adobe Acrobat™ Reader.
  4. Complete the form.
  5. Print the completed form.
  6. Package the completed form with your marked test socket or plaster model and mail to 14th Element Fabrication.
